Annual Report Form
For Charter Applicants


This is a specialized form of the Annual Report Form required by the Electoral College. Please fill it out to the best of your ability, indicating where questions are not applicable. There is a more detailed version for chartered local bodies- this version functions as an assessment form only. All information entered into this form is to be considered "Proposed" until actual approval of charter from the Electoral College.

This form has been tested with Internet Explorer and Netscape 4.0 and better. 

Please send an email to the Annual Report Secretary notifying her that you have filled out the form and pressed the submit button. She will email you and let you know if the information was received. You can also email her if you have problems with the form.
